Job Description

Job Title: Physical Therapist

Reports To: Chief Executive Officer

Supervises: None

FLSA Status: Non-Exempt

Bloodborne Pathogens: Not at risk

Position Summary:

Under the supervision of the Executive Director, BRH or other Supervisor, provides physical therapy services to patients in the hospice program.

Essential Job Functions:

  1. Performs initial and annual screenings/assessments as assigned and makes appropriate recommendations for services.
  2. Functions as a member of the interdisciplinary team. Collaborates on care planning and service allocation decisions.
  3. Completes member evaluations as needed and develops a plan of care. Evaluations include but are not limited to the following: gait assessment, balance, bed mobility, functional status, physical status, home safety and environmental assessments, restraint assessments, and wheelchair needs.
  4. Participates in care plan development and discharge planning for members.
  5. Provides regular updates to team on members’ status in therapy, including therapy discharge plans.
  6. Provides treatment to members and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals.
  7. Assesses need for adaptive and/or durable medical equipment (DME), makes recommendations to interdisciplinary team according to policy.
  8. In conjunction with primary care and the interdisciplinary team, assesses the need for specialized shoes and prosthesis according to policy. Assesses newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit as indicated.
  9. May assess the need for on-going skilled services and provides authorization for Physical therapies provided in a Nursing Home and with Contracted Agencies, following initial evaluation order by Primary Care.
  10. Completes documentation according to policy.
  11. Participates in committee meetings as needed.
  12. Provides training in body mechanics and use of assistive/adaptive devices to staff and caregivers.
  13. Educates staff, members, and caregivers on services available through physical therapy services as directed by the PT practice laws.
  14. Utilizes appropriate safety measures when providing care and performing other job duties.
  15. Supports Quality Improvement efforts.
  16. Protects privacy and maintains confidentiality of employee, member and sensitive agency information.
  17. Supports agency’s mission and values.
  18. Performs related non-clinical services necessary to maintain the efficient operation of the department.
  19. Maintains punctual, regular and predictable attendance.
  20. Works collaboratively in a team environment with a spirit of cooperation.
  21. Respectfully takes direction from supervisor/manager.
  22. Other duties as assigned.

Qualifications


Education and Experience:

1. Master’s Degree or Doctorate in Physical Therapy from an accredited program and current State of Virginia PT license required.

2. A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly population required.
